Hi Brian. I made a woden birthday card for my Bruvver about 20 years ago, I made it from 1/4 inch thick very darl oak. I stack cut two pieces about 7 x 5 with a nice fancy edge and hinged the two together using those tine hinges you see on small wooden boxes. I mananged to scroll happy birthday on the front and then used a stiff white card with the verse on in the centre using invisible tape. A friend was into card making and she helped with the insert. My brother was well pleased with it despite not having an envelope for it.
