As a retired Civil Engineer, I've had a lot of Health & Safety training - I cannot remember the figures but injuries from use of ladders was always quoted to be one of the highest ranking i.e. Very dangerous and very common.

Off topic but if we are recalling mad practices..............as cub scouts aged about 10, one week the leader decided it would be a good idea to clean the outside of the club house, we were all allocated sandpaper and told to sand an area of the asbestos hut, still remember to this day the dust coming off it like dandelion seeds floating in the bright sunshine.
 
My dad used to work at a steel rope maker, to temper the steel rods it was drawn through tanks of molten lead. They kept the heat in by manually sprinkling bags of asbestos on top....
