No but I was routing the ledge for the binding on a guitar when the bearing came off and I routed a hole in the back. After I had calmed down and thought a bit I tidied up the hole, cut a piece from the waste left over from cutting out the back and fitted it in. Luckily it was East Indian Rosewood and the figure was not strong in that area. It took the person I was making it for a long time to find the repair, when I asked him to look for it, and it didn't bother him.

Last check with guided bearings now? Check the screw.
 
Ouch! I'll be looking to make a similar fix...filling the area with some of the scrap Oak and then re-routing...with a different router!
 
I reckon the most important message in this post is the advice to step away and think things through when things go wrong.

Wish I had the wisdom to do that!
